Registration Form for Entry Test-48
Please pay attention to the following instructions before filling the form:
- Please provide absolutely correct information.
- Any wrong information, entered at this stage, may lead to disqualification from admission at any level.
- Please staple one recent Colour Photograph with the Permission Slip.
- Attach a photocopy of Student's FRC (Family Registration Certificate) or CRC (Children Registration Certificate commonly known as B-Form) issued by NADRA, with the Permission Slip too, please.
- The cited Permission Slip is to be produced on the scheduled day for appearing in the Entry Test.
- At the time of Admission, the FRC or CRC (B-Form) issued by NADRA (original) is to be produced for verification, otherwise the school management reserves the right to cancel the admission.
